On July 8, 2021 at 12:30 we held our Patient Care Assistant (PCA)/Administrative Support Associate (ASA) Recognition Day. Mary McDermott, MSN, RN, APRN, NEA-BC, Senior VP, Patient Care Services, CNO thanked everyone for contribution for getting through an unprecedented year. On May 11, 2021 the Age-Friendly Health Systems (AFHS) Committee presented the 3 North nursing staff with a token of our appreciation for performing the AFHS/4Ms pilot study. As evident in the graph below, 3 North's compliance with the daily review of the 4M's improved during the pilot. An ILearn Module is currently being finalized and education will be assigned the week of May 17. On June 7, 2021 the 4Ms will be rolled out to the following inpatient units: ICU, 5 North, 5 South, 3 North, 2 North, and 1 South.

Phelps Hospital Receives Exemplar Status
The nursing department is proud to announce it has received NICHE (Nurses Improving Care for Healthsystems Elders) Exemplar Status for the 5th year in a row.
Nurses Improving Care for Healthsystem Elders (NICHE) is a nursing education and consultation program designed to improve geriatric care in healthcare organizations. The NICHE program of NYU Rory Meyers College of Nursing provides resources for nursing and interdisciplinary teams to achieve organizational goals for the care of older adult patients.
The NICHE model demonstrates improved clinical outcomes, positive fiscal results, enhanced nursing competencies, community recognition, and greater patient, family, and staff satisfaction. NICHE model of care provides nurses with the knowledge and skills for delivering best practice in the care of older adults.
Phelps has been a NICHE hospital since 2013 and has earned and retained exemplar status since 2016.
Exemplar designation is the highest of 4 levels of achievement. To achieve Exemplar designation indicates a hospital has demonstrated ongoing, high level dedication to geriatric care and implements interventions and initiatives to continually improve the care of the geriatric population.
Requirements for exemplar status include implementation of the NICHE Geriatric Resource Model, evidence-based protocols on patient care units, use of age-sensitive standards, inclusion of patients, families and community based providers in planning NICHE initiatives and involvement in leadership roles regionally.
As an Exemplar NICHE hospital Phelps shows a tremendous commitment to meet a challenge of the times-quality care for the older adult.
NICHE hospitals offer evidence based, interdisciplinary approaches to care that promote better outcomes, positive patient experiences with increases in patient, family and staff satisfaction.
When a hospital continually works to Improve the Care for Healthsystem Elders…it also improves the care for all of its patients.

On September 2, 2020, we celebrated the graduation of our inaugural cohort for Nurse Mentor Entry to Leadership Track. A structured Nurse Mentor Program is part of Northwell’s nursing strategic plan. The goal of the mentoring program for leadership is to support the ongoing professional growth of newly-appointed directors, nurse managers, assistant nurse managers and nurse educators. The core components of the leadership track are: leadership role assimilation, succession planning, professional growth, career progression, and promotion of professional involvement.
The dyads worked together for a year and they all expressed appreciation for being part of the program. Some of the benefits that they identified includes being able to develop goals, opportunity to work on strengths and focus on what they need to accomplish. They also shared that they get to better know someone that they normally might not work with and they learn about unique situations from other departments.

Overall, 45% of our nurses participated in at least one of the sessions held during the visit from August 19-21. And we know that whether or not you had the opportunity to participate, you made a contribution and were well represented by your peers. All three appraisers expressed how impressed they were with the structures, processes and outcomes, driven by our Phelps nurses. They said that they could “feel” the sense of “family” in every session.
We expect to hear in about 2-3 months from the American Nurse Credentialing Center (ANCC) on whether we will be recognized as a Magnet® hospital.

We are excited to announce that Eileen Egan, RN, Esq., has been appointed as the Executive Director of Phelps Hospital Northwell Health. Two days after assuming the role of interim executive director of Phelps Hospital a few months ago, Phelps Hospital received their first COVID+ patient, and many more quickly thereafter. She demonstrated incredible grace under pressure, and her response was immeasurably valuable in dealing with a significant patient surge while keeping patients and team members safe. On June 3,2020, Phelps Hospital Northwell Health celebrated their very first DAISY Award Recipient. DAISY is an acronym for Diseases Attacking the Immune SYstem. The DAISY Foundation was formed in November, 1999, by the family of J. Patrick Barnes who died at age 33 of complications of Idiopathic Thrombocytopenic Purpura (ITP). The nursing care Patrick received when hospitalized profoundly touched his family.

On October 29, 2019 5 South celebrated the nurses who achieved their Progressive Care Certified Nurse (PCCN) Professional Certification.
*The PCCN examination is developed from the AACN Synergy Model for Patient Care. 80% of the test focuses on Clinical Judgement, while the other 20% focuses on Professional Caring and Ethical Practice. Some of the topics include neurology, hematology, caring practices, and clinical inquiry.

On September 25th, some members of the Safe Patient Handling Committee and Transfer Mobility Coaches (TMCs)attended the 1st Annual Northwell SPH Conference in Long Island. Phelps presented 2 posters: Transfer Mobility Coaches: Keeping our Patients and Employees Safe and Education Assessment to Address SPH Learning Needs. We are so excited to share with you that the TMC poster won the 2nd place.

American Society of Clinical Oncology certifies Cancer Institute locations
NEW HYDE PARK, NY —
Northwell Health Cancer Institute locations at Monter Cancer Center, Northwell Cancer Institute at Huntington, Imbert Cancer Center in Bay Shore and Northwell Health Cancer Institute at Phelps Hospital in Sleepy Hollow have each received certification from the Quality Oncology Practice Initiative Program, LLC (QOPI), an affiliate of the American Society of Clinical Oncology, Inc. (ASCO).
The QOPI certification is valid for three years and signifies Northwell’s oncology practices meet nationally-recognized standards for quality cancer care. QOPI is a voluntary self-assessment and improvement program to help hematology-oncology and medical oncology practices assess the quality of the care they provide to patients. About 300 practices nationwide are currently certified.
“We’re extremely proud to receive this certification from the ASCO’s QOPI® Certification Program,” said Tony Philip, MD, Director of Quality and Patient safety at Northwell Health Cancer Institute. “This recognition highlights our commitment to delivering outstanding cancer care to our patients across the region. Earning the certification took tremendous teamwork among our nurses, pharmacists, physicians and administrative leadership.”
QOPI® Certification Program
The QOPI® Certification Program for outpatient oncology practices is the first program of its kind for oncology in the nation. The seal designates those practices that successfully meet the standards and objectives of the program, which includes scoring above threshold on the key QOPI® quality measures and meeting chemotherapy safety standards established by ASCO and the Oncology Nursing Society.
QOPI® analyzes individual practice data and compares these to more than 170 evidence-based and consensus quality measures. To become certified, practices have to meet core standards in areas of treatment, including:
Creating a safe environment—staffing, competencies, and general policy; Treatment planning, patient consent, and education; Ordering, preparing, dispensing, and administering chemotherapy; Monitoring after chemotherapy is administered, including adherence, toxicity, and complications; Treatment planning; Staff training and education; Chemotherapy orders and drug preparation; Patient consent and education; Safe chemotherapy administration; Monitoring and assessment of patient well-being.

Judy Kennedy, BSN, RNC was just informed that her abstract has been accepted for a Poster Presentation at the Northwell Health's 25th Annual Nursing Research & EBP Conference to be held on May 23, 2019 at Leonard's Palazzo in Great Neck, NY. Judy's Evidence Based Project is "Bilirubin Surveillance newborns > 35 weeks gestation using the Transcutaneous Bilirubinometers." Click here to view all the presentations.
American Psychiatric Nursing Association (APNA) has accepted for Poster Presentation “The Effect of Coloring on Psychiatric Patient’s Anxiety”. The 2019 conference will be in New Orleans, LA October 2-5, 2019.

For the ninth year in a row, Phelps has received the Get with the Guidelines-Stroke "Gold Plus" Achievement Award for implementing specific quality improvement measures for the diagnosis and treatment of stroke patients as outlined by the American Heart/American Stroke Association. The “Get with the Guidelines-Stroke” program helps hospital teams provide the most up-to-date, research-based treatments with the goal of speeding recovery and reducing death and disability for stroke patients. Phelps earned this highest level award by meeting quality achievement measures including the aggressive use of medications and risk-reduction therapies at a set level for a designated period of time.

Cherry Fuentes wins First Prize at a Northwell Conference
On September 28th, Cherry Fuentes, MS-C, RN-BC, Education Specialist at Phelps, won first prize at Northwell’s“ Transforming Practice Using Evidence” conference for her poster entitled “New Graduate Nurse Mentoring Program.” In the words of Judy Dillworth, Magnet Director, her poster “exemplifies the wonderful mentoring program that she initiated at Phelps Hospital in response to conversations with her new graduate nurses.” For Mary McDermott, CNO and Senior Vice-President of Patient Care Services, Cherry’s mentoring program is “an important initiative for Phelps that has even been included in the strategic plan for Patient Care Services.”
Other benefits that the mentoring program offer are support and resources for new graduate nurses, the potential to improve RN to RN relationships, and the potential to improve clinical nurse retention.

For the eighth year in a row, Phelps has received the Get With The Guidelines- Stroke GOLD PLUS Achievement Award.

Phelps earned this highest level award by meeting quality achievement measures including the aggressive use of medications and risk-reduction therapies at a set level for a designated period of time. We have achieved 100% in 10 of the 12 performance measures.
We have consistently met our time targets and have also been awarded The Target Stroke Honor Roll Elite Plus for timely administration of TPA in less than 45 minutes 50% of the time.
It truly takes a multidisciplinary team to give our patients the highest quality of care.
